We are looking for a highly motivated full-time Registered Nurse Care Manager who is willing to think creatively and empathically to help our team change the way people access healthcare. \n\nWe seek a full-time Registered Nurse Care Manager to play a critical role in our whole-person, interdisciplinary care model by supporting patient-driven care plans to drive improved outcomes for individuals living with Serious Mental Illness, Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs. The Registered Nurse Care Manager will work in a team-based model with a lead care manager community health worker, behavioral health care manager, and nurse practitioner to contribute their clinical expertise towards improving the individuals quality of life through activities such as health education and complex care management. \n\nThis is a remote-forward hybrid position; while you will be asked to do in-person engagements in Santa Cruz, CA with another PairMate 1-2x/month, 90% of your work will be from home.\n\n\nKey Responsibilities:\n\n\nPrimarily work with and support a caseload of individuals with complex medical needs\n\nWork with individual to identify health/wellness goals and incorporate goals into a Shared Care Plan\n\nEducate individuals on medical and behavioral health conditions (including medication) to improve health literacy\n\nProvide medication reconciliation in collaboration with the individualsโs pharmacy\n\nProvide care management services such as coordinating prescriptions and completing prior authorizations\n\nTrack and assure that all required assessments and screenings are performed\n\nCollaborate with multidisciplinary care team to identify and address barriers to care\n\nIdentify clinical needs and triage escalations, providing brief interventions as necessary, with support from nurse practitioner clinicians\n\nCollaborate on care issues with Enhanced Care Management team by participating in systematic case reviews\n\nConsult with Enhanced Care Management team about clinical concerns or questions, provide educational training on chronic disease states, prevention, treatment, meds, and healthy living\n\nBuild trust and develop relationships with individuals experiencing homelessness, living with Severe Mental Illness/Substance Use Disorder, and living with multiple chronic conditions\n\nUse relationship-based strategies to engage individuals in care, understanding that many may have lived personal experiences causing them to be initially hesitant or distrusting of the health care system\n\nSeek to listen openly to individuals and meets them where they are โ understanding that adopting an โitโs not my fault but it is my problemโ attitude in all communication styles and approaches\n\n\n\n\n \n\nMust-Have Qualifications:\n\n\nMust hold active Registered Nurse license issued by the state of California\n\n* You are physically located in/near Santa Cruz, California\n\n\n* Field Ops requires you to maintain reliable transportation for engagement at clinic, community based organization, and health system partner locations\n\n* Virtual Ops requires a quiet, HIPAA compliant and internet connected space\n\n\n\n\n\n3+ years of experience working for a health plan or at-risk provider\n\n2+ years of experience in care coordination or case management\n\n* Bilingual โ English/Spanish\n\nMust have quiet and HIPAA-compliant at-home work environment with reliable Internet connection and cell phone \n\nStrong understanding of cultural fluency\n\nDemonstrated professional or personal lived experience working closely with individuals experiencing complex chronic needs, homelessness, or Severe Mental Illness/Substance Use Disorder \n\nStrong technical skills and comfort with technology innovation, past experience with CRM databases, basic Google suite, email, and video conferencing\n\nEmpathetic with a drive to reduce barriers to healthcare and social services for underserved communities\n\nA valid driverโs license and auto liability insurance\n\n\n\n\n \n\nPreferred Qualifications: \n\n\nA fantastic listener and skilled at โreading peopleโ - able to understand how others may be feeling or thinking based on nuances, uncomfortable silences, or questions they ask \n\nExcellent communication skills\n\nTakes accountability to resolve a patientโs needs to the best of his/her/their abilities \n\nComfortable building relationships with new people \n\nZest for problem solving, seeking answers, and thinking outside the box\n\nDetail-oriented and organized self-starter\n\nReliable and comfortable in an ever-changing environment\n\n\n\n\n \n\nBecause We Value You:\n\n\nSalary: $80,000 - $95,000/year\n\nComprehensive health, vision & dental insurance\n\n401k\n\nEquity compensation package \n\nMonthly $100 work from home expense stipend for your WFH days\n\nGas reimbursements for your on-site engagement days\n\nFlexible vacation policy -- take the time you need to recharge\n\nWe provide the equipment needed for the role\n\n\n* Opportunity for rapid career progression with plenty of room for personal growth!\n\n\n\n\n \n\nPair Team is an Equal Opportunity Employer.
