Remote Perinatal Mental Health Specialist
Key responsibilities of the role\n\nYour North Star: Provide direct patient care and care navigation by delivering evidence-based care that is founded in awareness, education, and prevention for the perinatal mental health issues affecting our patients. \n\nWe are hiring a creative, thoughtful, and skilled person who is eager to improve access to the highest quality perinatal mental and behavioral care, with a particular focus on reducing disparities. As a member of the clinical team, you would support our whole-person program of emotional, mental and behavioral care for pregnant people before, during, and after pregnancy. *This is a full time / salaried position - Hours are M-F, 8am-5pm*\n\n\nReporting to the Director of Therapy, your key responsibilities will include:\n\n\n* Providing one-on-one virtual care visits for Pomelo patients\n\n* Providing behavioral health and health related social needs screenings and appropriate response including diagnostic, therapy, and behavioral health interventions to pregnant and postpartum patients.\n\n* Performing ongoing psychosocial assessment of patients, providing reassurance and building rapport\n\n* Establishing individualized care plans to meet specific health needs based on evidence-based standards\n\n* Developing wellness goals by determining motivating factors and leveraging them for best possible health outcomes.\n\n* Identifying and addressing barriers that have been identified by patients.\n\n* Reducing care gaps (missed appointments, medication management, etc.) by frequent engagements with patient\n\n* Triaging of patients with urgent screening results and concerns to appropriate virtual and in-person emergency services\n\n* Collaborating with all involved teams and support services to ensure that patient and family needs are met.\n\n* Documenting all care/interactions in a timely fashion and escalating to appropriate multidisciplinary teams, as needed\n\n* Supporting group care, as needed\n\n\n\n\nWho you are\n\n\nIndependently licensed as an LCSW, LMFT, LPC or LMHC for at least 5 years\n\nLicensed in MO\n\nCompleted the NCMHCE, or both NCE & NCMHCE\n\nHave at least 120 hours of clinical supervision completed\n\nHave 5-8 years of experience providing therapy services in a hospital and/or healthcare practice\n\nHave experience with perinatal mental health care, possibly including: perinatal mood and anxiety disorders, substance abuse disorders, trauma-informed care\n\nAre enthusiastic for promotion of diversity, equity, and inclusion in all aspects of Pomeloโs internal and external programs\n\nPossess strong organizational skills and the ability to prioritize and follow through on multiple projects in a timely manner\n\nAre facile with email, spreadsheets, EHRs, and other commonly used software, and willingness to work with new software\n\nUnderstand the prevalence and role that birth inequity and structural racism plays in maternal morbidity and mortality\n\nAre professionally engaged\n\nHave experience working with clients eligible for Medicaid and/ or other public health programs\n\nMust be willing to cross license in other states where there is a company need (you'd be supported with this process by our licensing and credentialing team). \n\n\n\nBonus points if you have any of the following\n\n\nCertification in Perinatal Mental Health - PMH-C (would need to complete within 1 year of employment)\n\nMultiple state licenses \n\nExperience working with trauma patients\n\nExperience with cognitive behavioral therapy and EMDR\n\nExperience in case management and care navigation\n\nExperience providing virtual care\n\nExperience participating in group care\n\n\n\nWhy you should join our team\n\nBy joining Pomelo, you will get in on the ground floor of a fast-moving, well-funded, and mission-driven startup where you will have a profound impact on the patients we serve. In accordance with New York City, Colorado, California, and other applicable laws, Pomelo Care is required to provide a reasonable estimate of the compensation range for this role. Individual pay decisions are ultimately based on a number of factors, including qualifications for the role, experience level, geography, and balancing internal equity.
