The Case Coordinator is responsible for developing, in consultation with the client and caregiver, a comprehensive care plan to meet the client’s goals and for supporting the client and caregiver to manage their care budgets and required resources. The Case Coordinator supports the philosophy of client self-management and works within the framework of VON policies, procedures, standards and quality & risk management. The Coordinator is responsible for developing and supporting an individual support plan for clients with developmental disabilities in receipt of residential support from the Family Home Program. These services are funded by the Ministry of Children, Community and Social Services. The Coordinator reviews and assesses the needs of individuals requiring support related to all aspects of their support plan. The coordinator is responsible for providing a high quality, successful residential service for clients in our community. Key Responsibilities:\n* Interpret and completes current standardized assessment tools such as the interRAI CHA, supports Intensity Scale, Application for Developmental Services, or other assessment tools as appropriate.\n* Completes client assessments using conversation based methodology.\n* Develops and conducts a comprehensive therapeutic rapport.\n* Identifies and prioritizes environmental risk for client’s health and safety (shelter, food and running water, temperature control and overall condition of living environment). \n* Develops a comprehensive care plan (interpreting the Clinical Assessment Protocol and care planning).\n* Facilitates access to client identified services ensuring navigation to appropriate resources to support the care plan and goals.\n* Makes referrals to community and health resources as required.\n* With client consent, consults with partners in care service plans to ensure shared understanding of client needs and appropriate interventions/services to meet those needs.\n* Monitors client progress and adjusts care plans as required.\n* Reviews client care plan expenditures and supports clients to manage within the client care budget advocating for additional financial resources when needed for client care. \n* Accesses and reviews assessment.\n* Collects and reports relevant data to Manager on program outcomes as required by funder.\n* Adheres to the prescribed privacy policies and standards for Community Support Service or Developmental Services agencies including VON.\n* Positively and professionally represents all potential providers, external stakeholders, community agencies and service providers to clients while maintaining a depth of knowledge on the community resources. \n* Responsible to mediate and problem solve with multiple parties in a variety of contexts.\n* Coordinate and provide Family Home support for a caseload in accordance with program criteria, guidelines and directives\n* Responsible to ensure referrals and support are provided in emergency and urgent situations\n* Participate in the screening and matching of new Family Home Providers\n\n Common Responsibilities:\n* Promotes the goals and values of VON and their role as an integrated community care provider. \n* Promotes a safe and healthy workplace ensuring workplace conduct and activities are in accordance with the provincial Occupational Health and Safety Act and Regulations and compliant with the VON Safety Management System, including all Policies, Safe Work Practices and Procedures. \n* Abides by all VON policies and work practices.\n* Abides by all confidentiality and protection of personal information policies, regulations and practices and ensures appropriate safeguards are in place within their role.\n* Works in collaboration with other staff in a team approach to service delivery.\n\nExternal and Internal Relationships:\n* Engages frequently with Administrative Support, Community Support and Home Care program Coordinators and Manager.   \n* Networks and collaborates with all regional partners, Health Care and non-traditional providers and agencies.   \n\nEducation, Designations and Experience:\n* University Degree or College Diploma in Health or Social Services field.\n* Minimum of 3 years related experience in a home and community care organization or social services program including experience in case management and experience in building external and community relationships.\n* Demonstrated experience in completing validated common assessment tools and maintaining competency.\n* Broad working knowledge of community resources, including service and programs within defined catchment area.\n* Knowledge and skills in comprehensive assessment and the ability to maintain proficiency. \n* Working with volunteers and seniors an asset.\n\nSkill Requirements: \n* Strong attention to detail and accuracy. \n* Skilled in communication (verbal, written, listening), rapport building and relationship management.\n* Computer proficiency in Windows OS and Microsoft Office programs.\n* The ability to use analytical skills and understand/interpret client assessment data.\n* Ability to effectively manage multiple tasks, while prioritizing, meeting deadlines and working well under pressure.\n* Knowledge of social and developmental service sectors may be required \n* Demonstrated skills in leadership, team building.\n* Ability to educate client and families on available program resources.\n* Strong time management and organizational skills.\n* Ability to work independently and as part of a team.\n* Ability to accept and facilitate change.\n* Ability to maintain confidentiality.\n\nOther:\n* Available for weekend assessment coverage. \n* Must have access to a personal vehicle for business use and possess both a current driver’s license and proof of vehicle insurance.\n* A current and original copy of a satisfactory Criminal Records Check.\n* A Vulnerable Sector Search and/or a Child Abuse Registry Check may be required.\n* The successful candidate must be able to work flexible hours in order to meet client and family needs which will include evenings and weekends.\n* Ability to respond to emergency client situations 24/7 (on call)\n* The use of Personal Protective Equipment (PPE) may be required\n\n Working conditions and physical demands: This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in the role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements. \n\n#Salary and compensation\n $27 — $29/year\n
