We seek a full-time Community Health Worker to play a critical role in our whole-person, interdisciplinary care model, responsible for directly outreaching and engaging with individuals living with Serious Mental Illness/ Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs. We believe in the power of trust and relationships to successfully engage those who may have never received the kind of whole-health care that Pair Team can provide.\nKey Responsibilities:\n\n\n* Maintain ongoing caseload of individuals through the utilization of evidence based approaches to promote engagement and achievement of health goals\n\n* Use relationship-based strategies to support members with care navigation, understanding that many may have lived personal experiences causing them to be initially hesitant or distrusting of the health care system\n\n* Identify and connect individual to community resources necessary to promote individualโs health\n\n* Support the access and understanding of the health care system\n\n* Support the individual in enrollment of key assistance programs\n\n* Conducts periodic telephonic and SMS outreach to ensure timely follow-up to members \n\n* Work with member to identify, engage, and accomplish health/wellness goals \n\n* Identify and break down barriers ensuring individualsโ continuation with the program\n\n* Utilize external and internal online platforms to collaborate with team members and carry out daily tasks\n\n\n\nMust-Have Qualifications:\n\n\n* 3+ years of general work experience\n\n* 1+ year(s) of case management experience\n\n* Strong understanding of cultural fluency\n\n* High degree of empathy \n\n* Ability to work collaboratively in a multidisciplinary team\n\n* An eye for optimization\n\n* Organizational skills\n\n* Ability to remain patient when faced with adversity \n\n* Strong technical skills and comfort with technology innovation, past experience with CRM databases, basic Excel, Word, email, and video conferencing\n\n* Reliable travel to get around the Fresno community (as needed) \n\n\n\nPreferred Qualifications: \n\n\n* Demonstrated professional or personal lived experience working closely with individuals experiencing complex chronic needs, homelessness, or Severe Mental Illness/Substance Use Disorder \n\n* 2+ years of case management experience\n\n* Experience with motivational interviewing\n\n* Knowledge of medical terminology\n\n* Zest for problem solving, seeking answers, and thinking outside the box\n\n* Detail-oriented and organized self-starter who is a rockstar multitasker\n\n* Reliable and comfortable in an ever-changing environment\n\n\n\n\n \n\n \n\n \n\n#Salary and compensation\n
